Parent Request for Session Form
This form is for parents requesting a session. As soon as you submit this form, our respite workers will be able to see your request and pick up the session. Please make sure that you have contacted your primary Respite Workers before filling out this form. If they are not available, please fill out this form so we can find a backup to fill in for them. Please include if you prefer a female or male Respite Worker in the notes section, as well as other important information (i.e: one sibling also needs care, two siblings also need care, school pick-up, go to the zoo, take to Speech, etc).  The session will only show up in your Parent Portal once it has been staffed and an ARCC Center staff member has emailed you with the session details.
